Short Term Medical - Plan Details

Our Short Term Medical plans offer the following benefits:

• Up to $2,000,000 lifetime Maximum
• $50 co-pay for doctor’s office and urgent care center visits (maximum 3 per covered person; then 80% coverage)*
• $250 or $500 deductible options available.
• Choose any doctor or hospital
• Co-Insurance of 80% with a $5000 limit.
  This means, if your co-insurance limit is 80/20, we will pay 80% and you will pay 20% for covered expenses until you have met the $5000 co-insurance limit. After that, we will pay 100% up to the $2,000,000 lifetime maximum.

The policy will contain reductions, limitations, exclusions, and termination provisions. This Short Term Medical Plan is not available in all U.S. states or any other countries outside the U.S. and coverage and benefits may vary by state as well. Coverage can be selected for up to 6 months or up to 12 months. This is a temporary insurance plan. No benefit for any loss caused by or resulting from a Pre-Existing Condition will be covered. Expenses for the treatment of normal pregnancy or childbirth will not be covered or paid.
